Criteria

Endoscopic scoring systems can be used to evaluate the severity of inflammation in patients with inflammatory bowel disease and can indicate the response to medical therapy and long-term prognosis.[35]​ Two endoscopic scoring systems are commonly used in UC: the Mayo Endoscopic Score (MES) or the Ulcerative Colitis Endoscopic Index of Severity (UCEIS). The American College of Gastroenterology includes the MES as part of their definition of severity in UC.[45]​ However, scoring systems have not been fully validated.[45]

Mayo Endoscopic Score (MES)

  • 0 = normal mucosa, inactive disease

  • 1 = erythema, decreased vascular pattern, mild friability

  • 2 = marked erythema, absent vascular pattern, friability, erosions

  • 3 = spontaneous bleeding, ulcerations

Ulcerative Colitis Endoscopic Index of Severity

  • Vascular pattern

    • 0 = normal

    • 1 = patchy obliteration

    • 2 = obliteration

  • Bleeding

    • 0 = none

    • 1 = mucosal

    • 2 = luminal mild

    • 3 = luminal moderate/severe

  • Erosions and ulcers

    • 0 = none

    • 1 = erosions

    • 2 = superficial ulcer

    • 3 = deep ulcer
