Stratification for local BCC based on risk factors for recurrence. If a patient has any high risk factor, they should be categorised as high risk.

Low risk

  • Location and diameter: <2 cm on trunk or extremities

  • Clinical boarders: well defined

  • Primary versus recurrent: primary

  • Immunosuppression: none

  • Site of prior radiotherapy: no

  • Histological subtype: nodular, superficial, or other non-aggressive growth pattern

  • Perineural involvement: none

High risk

  • Location and diameter: ≥2 cm on trunk or extremities; or any size on head, neck, hands, feet, pretibial, or anogenital region

  • Clinical boarders: poorly defined

  • Primary versus recurrent: recurrent

  • Immunosuppression: yes

  • Site of prior radiotherapy: yes

  • Histological subtype: aggressive growth pattern

  • Perineural involvement: yes
