Bone marrow hypocellular, with no abnormal cells and no fibrosis.

At least two among the following:

  • Hemoglobin <10 g/dL

  • Platelet count <50 × 10³/microliter (<50 × 10⁹/L)

  • Absolute neutrophil count <1500/microliter (<1.5 × 10⁹/L).

Severity criteria[1][51][52]

Severe aplastic anemia (AA)

  • Bone marrow cellularity <25% or cellularity <50% with <30% residual hematopoietic cells

  • At least 2 among the following:

    • Absolute reticulocyte count <60 × 10³/microliter (60 x 10⁹/L) (using an automated analysis)

    • Platelet count <20 × 10³/microliter (<20 × 10⁹/L)

    • Absolute neutrophil count <500/microliter (<0.5 × 10⁹/L).

Very severe AA

  • Absolute neutrophil count <200/microliter (<0.2 × 10⁹/L)
